Abstract
body, which is particularly prone to various injuries. Osteoarthritis is a progressive degenerative joint disease
and one of the leading causes of disability worldwide, making early and accurate severity assessment crucial for OA progression monitoring, prediction of disease progression,
and timely interventions to prevent irreversible damage. Magnetic Resonance Imaging
has been widely used in the diagnosis of knee diseases due to its comprehensive imaging effect on bone, cartilage and ligament. However, automated OA analysis from knee
MRI remains challenging due to anatomical complexity, inter-patient variability, and the
presence of heterogeneous structural changes. In this work, we propose a cascaded deep
learning framework integrating hierarchical segmentation and severity classification. Initially, a coarse binary segmentation model was employed to identify and isolate the knee
region of interest from volumetric MRI scans, reducing irrelevant background information
and improving anatomical localization. These cropped volumes were then used as inputs
for a second fine-grained segmentation stage to extract clinically relevant knee anatomical
structures. To investigate segmentation efficiency, both 3D U-Net and 3D Swin-UNETR
architectures were explored, where the transformer-based 3D Swin-UNETR demonstrated
superior performance due to its enhanced ability to capture long-range spatial dependencies and multiscale contextual information. The refined segmented regions were then used
as the foundation for downstream classification tasks, including binary OA severity classification and multi-class classification of different osteoarthritis progression stages. Experimental results showed that the cascaded segmentation strategy significantly improved
region localization and enhanced classification reliability by focusing on structurally relevant quantitative biomarkers.
